university recognition: reporting forms
To participate in the University Recognition Program, you must complete and submit the required forms according to a designated timeline. The forms are combined in a packet that may be obtained at CES in Holtz Hall. To enroll in the program and receive a packet, you must meet with the CES University Recognition Liaision at 785-532-1689. Reporting forms are also available below.

Due Dates/Documentation Checklist (PDF)

Faculty Approval Form (PDF)
Submit your faculty approval form signed by a faculty advisor with a copy of your job description by the designated due date. This should be done prior to you beginning your internship/co-op experience.
Learning Objectives Agreement (PDF)
A minimum of three specific measurable goals, methods for achieving them and means to measure success. These will be submitted to your faculty liaison for approval and must be signed by yourself and your employment supervisor. They must be mailed to CES with original signatures.
Developing Learning Objectives (PDF)
Worksheet to help you write learning objectives.
Interim Report (online)
At least two brief paragraphs regarding your first impressions of your job and work setting. This can be submitted online.
Employer Performance Review (PDF)
Supervisor's review of your work. Must be mailed to CES with original signatures of student and supervisor.
Final Report (online)
Evaluation of the work site, ability to meet objectives, and overall impression of your experience. This can be submitted online.
