2012 FNP Annual Update: Big Changes in FNP Evaluation (ppt) The PowerPoint presentation OEIE presented at the 2012 Family Nutrition Program (FNP) Annual Update on October 3, 2012. This presentation covers the new impact evaluation instruments to be used for FNP/SNAP-Ed programming in Kansas. The Evaluation Decision Tree and more PEARS (formerly K-PICS) information was given to attendees in the handout (pdf).

FCS Program Evaluations: Focus Groups (ppt) The PowerPoint presentation OEIE presented in collaboration with Laurie Chandler of Shawnee County Extension office at the FCS Update on February 7, 2012. This presentation covers the when, why and how to conduct focus groups and uses examples of the Kansans Optimizing Health Program (KOHP) pilot focus group. The PowerPoint also includes the answers to questions the audience asked OEIE at the session.

Sample of an adaptable survey template (doc) This survey template and the accompanying instructions (below), provide an example of how a PFT can gather data for common questions asked across PFT programs, as well as for questions unique to an individual program. Sample of a survey form for short-term outcomes (doc) This survey form was developed by the Adult Development & Aging PFT to be used immediately after a program has been delivered and may be completed in paper format. Designed to be flexible and adaptable with the use of a question bank (doc), this form was developed to gather information about short-term outcomes (knowledge gain, with a question aimed at determining the intent to change behavior). Sample of a telephone form for medium-term outcomes (doc) This telephone form was developed by the Adult Development & Aging PFT to be used when contacting participants via telephone after a program has been delivered. Designed to be flexible and adaptable with the use of a question bank (doc), this form was developed to gather information about medium-term outcomes. Sample of an initial feedback form template with PFT question bank for short-term outcomes (doc) This initial feedback form template with question bank was developed by the Nutrition, Food Safety and Health PFT to be used immediately after a program has been delivered. Designed to be flexible and adaptable with the use of the PFT question bank and PFT standard questions, this form assists in gathering information about short-term outcomes across a variety of programs and action plans.
Sample of a follow-up feedback form template with PFT question bank for medium-term outcomes (doc) This follow-up feedback form template with question bank was developed by the Nutrition, Food Safety and Health PFT to be used when contacting participants via phone three to six months after a program has been delivered. This form was designed not only to be flexible and adaptable but also to complement the initial feedback form used for the program. With the use of the PFT question bank and PFT standard questions, this form assists in gathering information about medium-term outcomes across a variety of programs and action plans.
