Late Drop/Withdrawal Appeal Forms
Students requesting to drop or withdraw from a class or classes for which the drop or withdrawal deadline has passed are required to fill out the Appeal for Late Drop/Withdrawal Form. All students except 4th year Vet Med students will use the standard form.
Fourth-year DVM students are on a different academic calendar and need to work with the College Dean's Office directly when submitting the modified Late Drop/Withdrawal Appeal form.
Grade-Adjustment Forms
Incomplete Extension Request
A student can request the extension of an incomplete grade if they have experienced a significant, verifiable event outside of their control that impacts the completion of a course.
Academic Forgiveness or Fresh Start
Depending on the student's circumstances, they may apply for Academic Fresh Start or Academic Forgiveness to get a clean slate on their academic journey. Documentation and the form should be turned into Academic Standards Committee or College Dean's Office of their major.
