Club or Team Sport Verifications
Please follow these simple steps to help us make the process of certifying your members a smooth one.
If any of the requirements are missing or incorrect, the Office of the Registrar will not be able to complete the certification process for your club.
- Obtain the necessary certification form from the national governing body that oversees the sport. Please keep the following requirements in mind:
- The form should be typed, but if handwriting the information it MUST BE LEGIBLE
- DO NOT substitute first/last names with nicknames. Names should reflect what is seen in KSIS and affiliated with the students educational/academic record.
- The Wildcat (WID) 9-digit ID must be filled in.
- Leave all academic information blank - this information should only be completed by Registrar staff.
- If the form requests "The Family Educational Rights and Privacy Act" (FERPA) protected information, all listed students must sign the form to grant consent/permission to share their information.
- If the form does not directly mention FERPA then an additional consent process will be required along with the governing bodies form. The Campus {Faculty/Staff} Advisor should work directly with the Office of the Registrar for assistance.
- Student signature is required for consent.
- Identify on the Top of the Form the following items:
- Name of the Campus {Faculty/Staff} Advisor,
- E-mail and Phone in case there are questions or issues
- Forms WILL NOT BE RETURNED to any student, regardless if they are the sport chair, president, etc...
- Forms and questions may only occur between the Campus {Faculty/Staff} Advisor and Office of the Registrar in order to ensure full compliance with FERPA regulations.
- Completed forms may be brought over in-person or sent via e-mail - Be sure to indicate when dropping off the form when it is due and if you prefer to pick-up the form or have it e-mailed.

We ask that you allow for a minimum of 3-5 business days (excluding weekends and holidays) for completion of your form.
Once the verification/certification form has been completed, the Office of the Registrar will notify the Campus {Faculty/Staff} Advisor the form is ready based on the indicated preference in Step 4 above.
If you DO NOT have a Campus {Faculty/Staff} Advisor the Center for Student Involvement may act or support you in this process and should be who you connect with to ensure this process is successful.
BE AWARE: Just because a form indicates a university seal should be added, does not mean that the form is eligible for the seal. Refer to the University Seal website for additional information.
