Respirator Medical Clearance

Employees that must wear respiratory protection in the course of their work activities must be medically approved to wear respiratory protection by a licensed healthcare provider. To verify medical approval, the employee is provided a respirator clearance form that is filled out by the employer and provided to the healthcare professional. The clearance form lists the conditions of work, anticipated exposure type, and respirator required. The healthcare provider signs the clearance form, indicating if the employee is medically cleared for respirator use and listing any applicable limitations or conditions of approved use. All KSU employees using respiratory protection in the performance of their job must have documented medical clearance. Review other requirements of the Respiratory Protection Program.
