Chemical Container Labels
Secondary Chemical Container Labels
Chemicals transferred from a manufacturer's container to a secondary container must be labeled. Chemical containers for mixtures generated in the laboratory must also be labeled. A secondary container is any container that is not the original manufacturer container that a chemical was received in.
EHS has created the following labels for laboratories to use to label secondary containers. Laboratories may use any label for secondary containers that includes a full chemical name, hazard statements, date of preparation, and the name of the individual who prepared the chemical. For disinfectant containers, please also include an expiration date (or statement “made weekly”, “made daily”).
Template 1 (pdf) – Best simple label to have pre-printed (allows for prior printing and completion by hand in the lab)

Template 2 (pdf) – Best for mixtures (details chemical composition) and allows for prior printing and completion by hand in the lab
- Recommended for all labs that create mixtures to have printed copies for quick labeling by hand
Templates 1 and 2 are PDF fillable forms. You may fill them out prior to printing them to place on containers, or you may print them and then fill them out by hand as needed.
Template 3 (docx)– Completion on computer required but has GHS pictograms present.
Template 3 is a Microsoft Word document that shows GHS pictograms on the completed label. Please delete any pictograms from the label template that do not apply to the chemical you are labeling before placing them on secondary chemical container labels. These templates should be completed on the computer before printing to ensure that only the applicable GHS pictograms remain on the label placed on the chemical container.
Templates 1, 2 and 3 may be printed on Avery 5163 labels or printer paper and print 8 labels per page.
Labels may be resized to fit smaller or larger containers as needed for the laboratory.
All chemical container labels include the following information:
- Chemical name
- GHS hazard pictograms or hazard statements
- Name of responsible individual
- Date of preparation
Templates 2 and 3 have chemical composition percentages for use with mixtures.
