Additional information about vaccine medical exemption requests and Lafene Health Center

All Kansas State University employees must be fully vaccinated against COVID-19 by Wednesday, Dec. 8, to comply with the recent federal executive order. This requirement applies to all K-State faculty and staff as well as graduate and undergraduate student employees.

Federal guidelines allow for limited exemptions to the vaccine requirement based on medical status and sincerely held religious beliefs. Individuals requesting a medical exemption that precludes them from receiving the COVID-19 vaccine may submit a request for a medical exemption. Documentation from a medical provider is required. 

Currently enrolled student employees who have established medical care through Lafene Health Center while studying at K-State and who have no other primary care physician can request a medical provider at Lafene Health Center to complete the medical provider portion of the COVID-19 Vaccine Medical Exemption Request form.

Employees wishing to request a medical exemption will need to submit their request to Human Capital Services no later than Monday, Nov. 15, in order for the university to timely process the request and communicate back to the employee. Exemption requests will be approved or denied by Nov. 19.  This timeline allows time to get a single-dose Johnson & Johnson vaccine and be considered fully vaccinated by the Dec. 8 date.
