1095-C forms now available
The State Employee Health Plan has announced 1095-C forms have been released. You can save and print your 1095-C tax form by logging into your Member Portal.
Click on the Forms tab, then Tax Forms, then click on the 2022 1095-C. The form will open as a PDF for you to save or print. If you elected to have your 1095-C mailed, watch for it in your mailbox over the next several days.
