Lafene Health Center offers Johnson & Johnson vaccine clinic on April 12

Kansas State University’s Lafene Health Center will hold a COVID-19 vaccination clinic for current students, faculty and staff on Monday, April 12.
The clinic will be for the single-dose Johnson & Johnson vaccine. You must be at least 18 years of age to receive the Johnson & Johnson vaccine.
Appointments are required and should be made on the myLafene+ patient portal. Appointments will be available on a first-come, first-served basis. If no appointments are available, the clinic is full.
The clinic will be from 1-4 p.m. at the Chester E. Peters Recreation Complex. Parking is available at the northwest end of the Rec Complex parking area: lot C-1, lot C-2 or the east side of Bramlage Coliseum. No parking tickets will be issued at the Rec Complex during the vaccine clinic. Enter the clinic through the north entrance of the Rec Complex. Wear a face covering, maintain physical distancing and follow university health and wellness guidance during the clinic.
The day of the clinic you must bring your driver’s license or your K-State ID card for identification. You will also need to complete the COVID-19 Vaccination Consent Form, Risk Questionnaire and the Treatment Agreement/Notice of Privacy Practices. You should also review the Emergency Use Authorization for Johnson & Johnson (Janssen) and the Notice of Privacy Practices before your appointment. All of these forms can be found on the Lafene Health Center website. If you have questions regarding the content of the EUA, you will have an opportunity to discuss this with a nurse or health care provider before receiving the vaccination.
If you are symptomatic or have been identified as a close contact and are in quarantine, you are not eligible to receive the vaccine until you have completed your quarantine or are no longer symptomatic.
