KSIS Service Indicator Request Form

Add New Service Indicator

Please Note that as of 2022 holds for financial charges are no longer supported via an individual/departmental service indicator. Departments are asked to engage with the Cashier and Student Accounts Office when needing to place a charge on a student account. 

Requests to add or adjust holds for financial reasons will be denied. 

 

Please note: You must submit a new KSIS security form to be granted access to any new service indicators.
