1095C tax forms are now available

Employers are no longer required to mail printed 1095C as of the 2024 plan year unless an employee specifically requests one.

To print the form from the State of Kansas Employee Health Plan, or SEHP, Membership Administration Portal, follow these instructions:

1. Log in to the MAP Member Portal
• State Employees and Non-State Group employees. 
  
• ESU, KSU, KU, KUMC or PSU employees.
  
2. From the Account Overview page, click on the Forms tab.
3. On the Forms page, click on the Tax Forms tab. Scroll to the bottom of the list and click on the 1095C, 2024 link. The form will download automatically.
4. After the download, you should be able to open and print the PDF for your files.

Employees always have the option to request a copy from SEHP by submitting a communication form request in the MAP Member Portal.
