Request for a Non-Disclosure Hold of Student Directory Information
The Family Educational Rights and Privacy Act (FERPA) of 1974, as amended, designates certain information related to a student as "Directory Information." FERPA gives the University the right to disclose such information to anyone inquiring without having to ask a student for permission, unless the student specifically requests in writing that all such information not be made public without written consent, except by the National Student Clearinghouse to loan guarantors.
Kansas State University has designated the following as "Student Directory Information:"
- Name
- Local address and telephone number
- Permanent address
- E-mail address
- Date and place of birth
- Photograph or likeness
- College
- Curriculum
- Enrollment status (full/part-time)
- Classification
- Dates of attendance at K-State
- Awards and academic honors
- Degrees and dates awarded
- Most recent previous educational institution attended
- Participation in officially recognized activities and athletic teams
- Height and weight of student athletes
Consider carefully the consequences of any decision made by you to withhold your "Student Directory Information," as any future requests for such information will be refused. Examples of, but not limited to, potential impacts are: no acknowledgement of your attendance at K-State to potential employers, no verification of degrees to requestors, no printing of your name in the commencement program and no press releases pertaining to graduation and/or honors. K-State will honor your request to hold "Student Directory Information" but cannot assume responsibility to contact you for subsequent permission to release it. Regardless of the effect upon you, K-State assumes no liability for honoring your instructions that such information may be withheld.
If the form is submitted before you have left the university (i.e. graduated, transferred, withdrawn), the "Student Directory Information" hold will remain in effect until the Office of the Registrar receives a written request to release the hold.
If you wish to withhold the disclosure of all "Student Directory Information" items, the Request for a Non-Disclosure Hold of Student Directory Information form must be completed and submitted to the Office of the Registrar, 118 Anderson Hall (208 College Center, Salina). Within 24 hours from the submission of the form, the hold will restrict the release of all "Student Directory Information" in the K-State student information system, KSIS. The form must be received in the Office of the Registrar 15 calendar days from the beginning of the term or your "Student Directory Information" will be published in the K-State Campus Phone Book. Submitting the form after the 15th calendar day could result in your "Student Directory Information" being released to other University publications and databases.
Release of a Non-Disclosure Hold of Student Directory Information
If you have previously submitted a Non-Disclosure request, but now wish to disclose your information (i.e. release the non-disclosure hold), then you may fill out the Release of a Non-Disclosure Hold of Student Directory Information form.
