Faculty and Staff Forms


For each form that requires a 
student signature, we encourage you to work with students that do not have the ability to print out the form, sign it and re-scan back to you.  Instead, we would encourage you to consider accepting the form electronically via the students' official K-State email address and referencing or attaching that to the form in question. 

You may then work the form through whatever additional protocols are available via the University, College or Academic Department processes in order to ensure completion and successful submission for approval and processing.  Due to the limited operations being offered we highly recommend that all PDF forms are funneled through university email protocols for proof of signature.

All forms are for College & departmental use only - any form submitted by a student will be denied/rejected.

**If you are unsure if an evaluation has already been done review the Transfer Equivalency Database to confirm before submitting a form.

  • Instructor is no longer employed with KSU and cannot make the chagnes in KSIS

Subject Line: Report Term/Year Students for Non-Attendance in SSC Navigate

This is the new process utilized to report drops for non-attendance.  Search for this Subject Line in your email as well as your Junk email folder.  Every instructor of record as of the first day of classes will recieve this email for reporting non-attendance.

Student Services Fee Waiver
